Patients are given every opportunity to complete treatment planned for them in this practice.
We do require advanced notice of at least 24 hours notice whenever possible as a courtesy to the Doctor and/or Hygienist you are scheduled to see that day. We do know that on occasion emergencies as well as illnesses do occur that limit patients abilities to keep scheduled appointments; when these arise considerations and accommodations will be made.
Our policy is that after two consecutive broken and/or late cancelled appointments patient termination from the practice can and will be enforced. In addition, after 4 broken and/or late cancelled appointments (whether consecutive or not) within a 6 month period patient termination from the practice will be imposed. For Non-Medicaid patients there is a $50 fee for all broken and/or late cancelled appointments.
We do our best to be accommodating but the Doctor and Hygienist do work by appointment only, so please be considerate and respectful of their time. We request that you sign below indicating that you understand the policy as present above. If you have any questions, please don’t hesitate to ask.
